Judging by your post, you are not using any modpacks, so I will base my answers on that.

Pressing the spacebar should put you into 3D sailing mode.

When you are sailing around an island in 3D sailing mode, press Enter and use the menu that shows up to go to the ammo switching commands.

If you are in map-sailing mode, you should first go into 3D sailing mode by pressing the spacebar. Then, when you are in 3D sailing mode, pressing Enter will bring up the quick action command menu, where you will find the "Sail to port" commands.

If you mean any patches made by Akella: There are only some patches for the Russian and Polish versions, I think. Not for any English or European versions.
However, a lot of the fixes from these patches did make it into Build 12, the modpack for PotC. This modpack does work properly on European and English versions and is a great improvement to the game. It fixes many bugs and adds a lot of new features. This modpack is a must-have for any PotC player who wants the best out of his/her game. It is freely available for download from <a href="http://www.piratesahoy.net/component/option,com_docman/task,cat_view/gid,59/Itemid,46/lang,/" target="_blank">this</a> page. Once you played a bit stock (unmodded) PotC, get Build 12 and prepare to be impressed. <img src="http://www.piratesahoy.com/forum/style_emoticons/<#EMO_DIR#>/icon_mrgreen1.gif" style="vertical-align:middle" emoid="

There have been many more mods made since the release of Build 12. Most of them are in the Post Build 12 modpack, which is available from <a href="http://pieter.piratesahoy.net" target="_blank">Pieter's Pirates Page</a>. Although there are many great new mods in there, there are also several bugs still in there that we are trying to fix. So if you want to have the fullest gaming experience and don't mind a few additional bugs, get the Post Build 12 mods too. Otherwise just get the almost bug-free Build 12 mod.
You are right about the manual. It's isn't exactly great. And some of the things in the manual are downright lies that are not true in the game. <img src="http://www.piratesahoy.com/forum/style_emoticons/<#EMO_DIR#>/piratesing.gif" style="vertical-align:middle" emoid="
